Minimum qualifications:
- Registered, appropriately qualified and in good standing with the General Social Care Council.
- Ability to meet liability insurance criteria:
- Valid driver’s license, available personal vehicle, and proof of current vehicle insurance;
- Concise verbal, written, and organisational skills.
- Leadership and Supervisory skills
Desired qualifications:
- Extensive assessment skills and at least two years of domiciliary experience.
- First aid certificate.
- Three years social work relevant experience.
- Two years of management experience.
Physical requirements:
Demonstrated ability to effectively communicate and interact with families, co-workers, and all supervisors; Ability to proficiently follow and process written and verbal instructions in the English language; Physically and mentally fit for work to be performed, or potentially to be performed;
Job description:
The Social Worker is responsible for assessing and reassessing the home care needs, including the physical, cognitive, psychosocial, and environmental needs of clients and their families, with due consideration to the safe delivery of care. The Social Worker shall supervise all in-home care providers by making quarterly visits to each client’s place of residence. The Social Worker shall document all supervisory visits. The Social Worker shall also conduct six-monthly assessments of the client’s condition, progress and response to services and document these visits in the client’s service record. The Social Worker will develop an appropriate and individual care plan, collaborating with other health care professionals and/or providers as appropriate or required and is responsible for making recommendations to/counselling clients regarding self-care and evaluating client responses to care provided. Acting as the care coordinator for assigned individuals/ families, the Social Worker initiates referrals as necessary, supervises and directs Home Companions, Caregivers, other Care Workers and families, regarding client care. The Social Worker shall provide Formal Supervision, to subordinates, as required by the Registered Manager
General responsibilities:
- Supervise all in-home providers by making supervisory visits to each client’s place of residence at least every six months and provide written documentation of all such visits.
- Conduct and document competency demonstrations of all in-home care providers when needed and provide Formal Supervision, as instructed.
- Visit each client’s residence at six-monthly intervals or sooner, if circumstances necessitate, to assess the client’s condition, progress and response to services provided and provide documentation of the reassessment to be maintained in the client’s service record
- Adhere to agency infection control and safety procedures by performing and documenting observations, procedures, and interventions in the home according to the established care plan in a safe and competent manner.
- Effectively communicate and interact with clients, care workers and families in a supportive and positive manner.
- Management of client caseload and associated or other delegated work responsibilities in an effective manner within the resources of the agency and according to agency standards.
- Interpretation of ComForcare programs and policies as appropriate to care workers, clients and members of the community.
- Participates in the continuing commitment for quality care, including attendance at mandatory agency staff meetings.
Assessment responsibilities:
- Initiation of the Care Plan including the securing and updating of complete and accurate health information on a timely basis.
- Development of Care Plan based on an assessment and identification of the environmental, behavioural, physical, and psychosocial needs of the client/family.
- Teach, coordinate, and supervise family members and other agency personnel (caregivers, companions, nurses, and administrative staff) in the care of the client.
Reassessment responsibilities:
- Evaluate client, family, and agency care worker(s) responses and compliance with care. Update the Care Plan to reflect changes in the client/family needs, abilities, and resources every six months.
- Interpret and document, on a timely basis, in-home care provided to ensure compliance with all regulatory bodies, including all client clinical records.
