Once we understand your specific situation, we will discuss options available to your loved one, describe our services in greater detail and schedule a no-cost, no-obligation home visit to conduct a more thorough needs assessment.
A Home Visit with You, Your Loved One and Family Members
We offer families the opportunity to get to know us better during a no-cost, no-obligation, home visit with one of our staff members. This visit can take place in a private residence, care facility, hospital or rehabilitation center.
During this visit, we will further evaluate the needs of your loved one and your family, set goals for care, conduct risk assessments to ensure safety and discuss costs and available funding options.
Person-Centred Assessment and Care Planning
Once all parties agree on an appropriate level of care and to begin home care services, one of our experienced members of staff will visit to deliver the full personalised care plan and assessments and go through in detail with you to ensure that the care plan reflects the care that you want an the outcomes you desire.
The care plan will be kept in the home and followed by the caregiver during every visit.
We want your care experience to be successful. Key to that success is the development of a warm and trusting relationship between your loved one and his or her caregiver or caregiver team.
We consider what we have learned about your loved one during our conversations, visit and assessment and then carefully match your loved one with a caregiver or caregiver team that best matches his or her personality and needs.
Client Record Management and Scheduling
Throughout the process, we record your details and needs from the care plan and assessments into our proprietary care software.
This enables us to match your loved one with an appropriate caregiver or caregiver team, schedule visits according to your needs and monitor each shift for appropriate coverage.
Follow Up, Quality Checks and Reassessments
To ensure that you are receiving the highest quality care, we will call you regularly, during the first and second weeks and every month thereafter to ask if you are satisfied with the caregiver and services.
We also provide home visits to spot check our caregivers and improve the services delivered. To continue meeting your goals we will perform reassessments whenever they are required, at least six monthly, to ensure that the care we provide, is the care that you need and we are working towards and succeeding in meeting your goals.

We are available 24/7/365 and employ a large staff of caregivers to ensure uninterrupted service.
We provide on-going training and education for our caregivers so they are the most knowledgeable in all areas of home care.
If your loved one’s needs change and extend outside the scope of our care, we will coordinate resources to meet those needs.Top of Page
